Our heart failure clinic supports referring GPs at the earliest stages of disease identification and enables close collaboration with heart failure specialists during the patient’s diagnosis, treatment and management.

 

1. Heart failure identification
We support you with a dedicated Heart Failure Clinic referral process. This enables you to identify heart failure earlier in your community and ensures prompt access to expert care.

 

2. Confirmation of diagnosis
Once referred your patients will be enrolled in the clinic if they have a confirmed diagnosis of heart failure.

 

3. Simplified referrals
We encourage you to refer patients who show clinical signs and symptoms of heart failure.

Once referred, your patient will receive an ECG, ECHO and pathology tests including NT-proBNP. Patients diagnosed with heart failure will be enrolled in the Heart Failure Clinic.

With our personal support program, management of patients has been designed specifically for improving heart failure outcomes. Patients receive a personalised plan that enables continuous management and ensures medical compliance through regular monitoring, phone contact and face-to-face consultations with their health team and a dedicated patient counsellor.

Additionally, patients are provided with a comprehensive suite of educational materials, helping to better prepare them for living with heart failure and achieving their treatment goals.

We believe in a co-management approach to heart failure care, improving collaboration between healthcare professionals and clinics, and ensuring heart failure patients are managed in the most optimal way throughout their care.

Co-medical management
We work closely with GPs, specialists and allied health professionals to determine who needs to be involved at each stage of care.

 

Symptom monitoring and alerts
We track your patient’s condition and alert the relevant treating physicians when symptoms change.

 

Detailed clinical reports
We provide regular, comprehensive reports on the patient’s clinical status to the co-management team.
